Insurance Information

Medicare covers a portion of prosthetic eyes and related services. Prior to your fitting, a letter of medical necessity from your referring Ophthalmologist is required. Doctors Written Orders (DWO) or Standard Written Orders (SWO) must be obtained prior to services rendered by our office.

• New York State Medicaid:
  Strauss Eye Prosthetics, Inc. is a provider of service with the New York State Medicaid Program.
  

Private Insurance:
Most insurance pay at least a portion of our fees.

Therefore, it is the patient’s responsibility to provide the required information to our staff for the purpose of investigating your particular insurance benefit. Our staff is available and willing to help with insurance questions, claims submission, etc. Because we are not in/network with all insurances, the patient is responsible for fees at the time of service. Strauss Eye Prosthetics, Inc. does not have a financing plan. However, for your convenience, we do accept VISA, MasterCard and Discover credit cards.
